| Further conditions with respect to Swiss Re Loss Control Credit: |

When attending an approved Loss Control Seminar, be sure to complete & turn in the attendance verification form that will be provided at the end of the seminar so it may be submitted to Swiss Re for your agency’s underwriting file.
No Claims are made against your agency in each of the three years following the attendance at the seminar.
If there are no claims made during the three year discounted term, the insured may qualify for credit by attending another seminar during the third year of the discounted term and prior to the policy period expiration date.
If a claim is made against the insured during any of the three years of the discount term and the agency qualifies again prior to the next renewal date by attending Loss Control seminar(s) as described above, the loss control credit will be reinstated and the three year loss control period will begin to run again. This is one-time opportunity; additional losses during the policy period results in the loss of the credit at the next renewal date.
For the purpose of this program, a claim is defined as an Errors & Omission incident or situation for which a payment for loss and/or expense is made by either the Corporation (Swiss Re) of the insured or if a loss reserve on a reported incident is established by the Claims Department of the Corporation (Swiss Re). Reporting of an incident where no reserve is established and no payment is made will not be considered as a claim for purposes of this program. Any time or expenses for the Swiss Re Insurance Corporation staff does not constitute a payment for loss or expense.
