| STATE EXAM INFORMATION: Thomson Prometric will administer the state licensing exam. You’ll receive Thomson Prometric Test Center location information from MAIA. To register for the licensing exam, call Thomson Prometric at toll free (800) 741-9380 or on the web at www.experioronline.com The cost of the exams will be $59.00 per exam -- if you take two exams at the same time, you may register and schedule for both exams for one $59.00 fee. Payment for the exam is due upon registration. Fee may be paid by cashiers check, company check, money order, and MasterCard or Visa. Make checks payable to "Thomson Prometric". Please put your Social Security number on the check. Personal checks and/or cash are not accepted. Examination registration fees are non-refundable or transferable and expire in 90 days.
